Abstract
Purpose: To present the refractive outcomes, comparative defocus curves, proposed mechanisms of action, and methods to mitigate IOL-specific complications of the Crystalens intraocular lens.
Methods: Literature review.
Results: Distance-corrected intermediate vision with Crystalens and its toric version, Trulign, average 20/23 and distance-corrected near at 20/40, which are favorable outcomes compared with non-presbyopia-correcting IOLs. Evidence supporting accommodation as the principal mechanism of action of Crystalens is limited, and it may be better classified as an extended depth of focus IOL. The IOL provides high levels of contrast sensitivity and low incidence of photic phenomenon. The occurrence of Z-syndrome, consequent to capsular contraction, causing asymmetric flexing of the haptic-optic junctions, is reviewed along with methods to mitigate and manage this potential complication.
Conclusions: Crystalens is an aberration-free IOL that provides extended depth of focus and is immune to the optical effects of IOL decentration. It provides a high contrast retinal image, does not split light to multiple foci, has low incidence of photic phenomenon, and is particularly useful in patients with mild macular pathology, glaucoma, or post-refractive surgery where diffractive optics can be problematic.
