Gender-Related Differences in Transcatheter Aortic Valve Implantation

Abstract

Transcatheter aortic valve implantation (TAVI or replacement (TAVR)) has become the standard of care for patients with severe symptomatic aortic stenosis who are at increased risk for surgical aortic valve replacement (SAVR). Currently, trials are ongoing looking at those patients who have been deemed low surgical risk. Research has shown there exists a difference in the way male and female hearts remodel when exposed to chronic pressure overload secondary to conditions such as AS. Unlike trials conducted on most other cardiovascular disease states, females represent nearly half of enrolled patients in TAVR studies. While current data demonstrates that female gender is a risk factor for adverse outcomes following SAVR, the impact of gender on outcomes has suggested that this adverse relationship does not exist for TAVR. This chapter will review the data on gender differences in AS and treatment outcomes.
