Abstract

With the advance of lung allogeneic transplantation, more and more transbronchial biopsies are encountered by surgical pathologists, mainly for the purpose of the monitoring and diagnosis of allograft rejection. Surgical lung biopsy and explanted allografts are also occasionally encountered. This chapter illustrates the classification and grading scheme of pulmonary allograft rejection defined by the 2007 International Society for Heart and Lung Transplantation (ISHLT) formula, which is a revision of the 1996 ISHLT working formulation. The grading and histologic criteria for acute cellular rejection, airway inflammation, and chronic airway and vascular rejection are listed in Table 8.1.
