Abstract

Older patients reside in a wide variety of settings; these residential locations influence their ability to participate in their care. Transitions between these locations require different documentation, different orders, and care coordination. The vast majority of older patients reside in their homes. Care from family or community supports such as visiting nurse agencies or Area Agencies on Aging may be required. Nursing homes continue to offer short term rehabilitation or long term care. Assisted Living Facilities vary in supports provided. Special continuity of care is provided through Programs of All-Inclusive Care for the Elderly (PACE), or by Continuing Care Communities.
