Abstract

Increasing populations of children are developing gallbladder disease and there is a greater awareness of pancreatitis in children. This has necessitated improved diagnostics and therapeutics for pancreaticobiliary disease in the pediatric population. Endoscopic retrograde cholangiopancreatography (ERCP) has become a widely used technique in pediatric patients of all ages to both diagnose and treat a similar spectrum of diseases as in adults. ERCP is performed by both, adult gastroenterologists as well as a cohort of pediatric gastroenterologists with advanced training. The earliest ERCPs were performed mostly for diagnostic purposes, but with increased access to MRCP, ERCP is now largely performed for therapeutic indications in the pediatric population. Unique equipment may be necessary in smaller children, but standard “adult” equipment is frequently used in children over 10 kg. Although young children should be evaluated for gallstones or hereditary pancreatitis, a congenital disorder such as a choledochal cyst or pancreas divisum should be suspected depending on the clinical scenario. While less common, malignancy of the biliary system and pancreas occur in children and may need endoscopic therapy.
