Medical Management of the Pregnant Patient

Abstract

Diabetes in pregnancy has important implications for both maternal and fetal health. The increasing prevalence of diabetes requires that providers caring for women of childbearing age understand how to counsel and manage women with diabetes. In women with type 1 and type 2 diabetes mellitus (DM), pregnancy outcome can be improved by pregestational intervention to insure euglycemia during conception and the embryonic period. In gravidas whose glucose intolerance is discovered during pregnancy [i.e., gestational diabetes (GDM)], glucose surveillance during pregnancy and diabetic and other metabolic screening following pregnancy positively impact fetal and maternal health. The aim of this chapter is to describe the effect of pregnancy on glucose metabolism, discuss the clinical importance of diabetes in pregnancy, summarize screening and diagnostic testing models for GDM, and highlight important clinical issues that impact on pregestational and gestational diabetes management.
