Abstract
This case illustrates some of the challenges associated with performing the MitraClip procedure in a patient with a very large left atrium. These include modifying the usual landmarks used for selecting the ideal location for the transseptal puncture. In this case, the transseptal puncture was at the thick part of the septum, and required sequential balloon dilatations before the steerable guide would cross.
