Controversies in the Treatment of Women with Early-Stage Epithelial Ovarian Cancer

Controversies in the Management of Gynecological Cancers

Abstract

The management of women with apparent early-stage epithelial ovarian cancer presents a wide range of interlinked diagnostic and therapeutic challenges and controversies including surgical staging and surgical and medical treatments. We provide a critical overview of the most debated and controversial aspects of this challenging disease. In surgery, we focus on confidence in initial staging and the impact of under-assessment and undertreatment, the appropriate degree of surgical radicality, and need for lymphadenectomy, as well as the importance of fertility preservation in women with very early disease. We review the evidence for the need for adjuvant therapies and the optimum regimens particularly addressing the impact on younger women and long-term treatment sequelae in survivors.
