Zusammenfassung
Die vorliegende Kasuistik beschreibt ein Kind, das aufgrund unklarer abdominaler Symptome appendektomiert wurde. Postoperativ manifestierten sich klinische Symptome einer Beckenvenenthrombose. Radiologisch stellten sich ein Thrombus in der V. cava inferior sowie eine Thrombose der Vv. iliacae bei partieller Hypoplasie der unteren infrarenalen Hohlvene dar. Molekulargenetisch wurde ein heterozygoter Protein-S-Mangel als endogener Thromboserisikofaktor nachgewiesen. Die Bedeutung der bildgebenden Diagnostik bei unklaren abdominalen Symptomen wird bestätigt.
Abstract
This case report describes a child who underwent an appendectomy after unclear abdominal symptoms. Postoperatively, the patient suffered from thrombosis of the iliac veins. Magnetic resonance imaging showed an intravascular thrombus of the inferior caval vein, the occlusion of both iliac veins, and a hypoplastic malformation of the infrarenal caval vein. The molecular genetic examination revealed a heterozygous protein S deficiency as an endogenous risk factor for thrombosis. This case demonstrates the importance of image-based diagnosis in unclear abdominal symptoms.
