Kutanes Plattenepithelkarzinom beim alten und geriatrischen Patienten

Zusammenfassung

Das kutane Plattenepithelkarzinom ist der zweithäufigste maligne Tumor der Haut nach dem Basalzellkarzinom. Die chronische und kumulative UV-Exposition stellt den entscheidenden ätiologischen Faktor dar. Durch eine vollständige chirurgische Exzision sind ca. 95 % der kutanen Plattenepithelkarzinome kurabel und die Prognose ist somit in den meisten Fällen sehr gut. Bei einem geringen Anteil der Patienten tritt eine primär lymphogene Metastasierung ein, die mit einer schlechten Prognose verbunden ist. Mehrere patienten- und tumorspezifische Faktoren, die die Identifikation dieser Patienten ermöglichen, konnten ermittelt werden. In Fällen einer inoperablen Metastasierung sollte eine systemische Therapie möglichst im Rahmen einer klinischen Studie erfolgen, da derzeit keine ausreichend evidenzbelegte systemische Therapie zur Verfügung steht. Cisplatin und 5-Fluorouracil sowie Cetuximab stellen weitere Therapieoptionen dar.
