Zusammenfassung
Neuroendokrine Neoplasien sind mit 80 % die häufigsten Neoplasien der Appendix. Meist handelt es sich um (hoch differenzierte) neuroendokrine Tumoren (NET). Diese werden häufig inzidentell gefunden, sind prognostisch meist günstig und zeigen in der Regel keine funktionelle (endokrinologische) Aktivität. Sehr viel seltener findet sich im Appendix das Becherzellkarzinoid, ein aggressiver Tumor, der endokrinologisch ebenfalls meist stumm ist, die meisten Appendix-NEN führen nicht zu tumorspezifischen Symptomen. Da es sich bei den neuroendokrinen Neoplasien der Appendix meistens um eine postoperative Zufallsdiagnose nach stattgehabter Appendektomie handelt, bezieht sich das diagnostische Vorgehen auf ein postoperatives Staging. Welche Diagnostik indiziert ist, hängt weitestgehend vom histologischen Untersuchungsergebnis ab, wobei die korrekte T-Klassifizierung von entscheidender Bedeutung ist. Auf Basis der unten aufgeführten Risikostratifizierung werden auch Therapie und Nachsorge festgelegt..
