Zusammenfassung
Die membranöse Glomerulonephritis ist durch subepitheliale Immunkomplexe mit Komplementaktivierung definiert. Vermutlich als Folge dieser Ablagerungen kommt es zu einem Verstreichen der Fußfortsätze der Podozyten und einer großen Proteinurie. Auslöser ist wahrscheinlich die Bildung eines Autoantikörpers gegen ein podozytäres Antigen. Grundsätzlich wird zwischen einer primären und einer sekundären membranösen Glomerulonephritis unterschieden. Die membranöse Glomerulonephritis manifestiert sich meist durch ein nephrotisches Syndrom und durch Ödeme. Neben Anamnese und körperlicher Untersuchung gehören zur Diagnostik Laboruntersuchungen (u. a. Quantifizierung der Proteinurie, Urinstatus, Nierenfunktion) sowie ein Ultraschall der Nieren und eine Nierenbiopsie. Die Therapie besteht unter anderem aus einer Gabe von ACE-Hemmern oder Angiotensin-Rezeptorenblockern und einer Salzrestriktion. Die immunsuppressive Therapie folgt dem Ponticelli-Schema. Spontanremissionen kommen in ca. 30 % der Fälle vor.
