Zusammenfassung
Die Pneumonie ist eine Infektion des Lungenparenchyms und Infiltrat mit begleitenden klinischen Zeichen der bronchoalveoläaren und systemischen Inflammation definiert durch ein neu aufgetretenes radiologisch nachweisbares Infiltrat. Pneumokokken sind der häufigste Erreger der ambulant erworbenen Pneumonie. Respiratorische Symptome sind Husten, Dyspnoe oder Tachypnoe, Rasselgeräusche oder Bronchialatmen. Prognostische Scores wie der CRB-65 können als Entscheidungshilfe für oder gegen eine stationäre Einweisung dienen. Die antibiotische Therapie der ambulant erworbenen Pneumonie erfolgt entsprechend einer fünfgliedrigen Risikostratifikation: ambulant mit und ohne Risikofaktoren, Normalstation sowie Intensivstation mit und ohne Risiko für eine Infektion durch Pseudomonas aeruginosa. Hauptsäule der Therapie ist in allen Klassen ein Betalaktam mit guter Pneumokokkenwirksamkeit. Immobile Patienten mit ambulant erworbener Pneumonie sollten prophylaktisch antikoaguliert werden.
