Zusammenfassung

Das hepatozelluläre Karzinom (HCC) ist die größte Gruppe der primären malignen Leberkarzinome. Meist entsteht das HCC auf dem Boden einer Leberzirrhose. Weitere Risikofaktoren sind Hepatitis-B- und Hepatitis-C-Virusinfektion, Alkoholkonsum, metabolisches Syndrom und die Kontamination von Nahrungsmitteln mit Aflatoxin B1. Die Klinik ist bei einem HCC unspezifisch und wird meist durch die zugrunde liegende Lebererkrankung dominiert. Risikopatienten sollte eine regelmäßige Früherkennungsuntersuchung angeboten werden. Bei der Diagnose spielen bildgebende Verfahren und die Biopsie die wichtigsten Rollen. Die Therapie des HCC orientiert sich im Wesentlichen an der Klassifikation nach BCLC und besteht aus kurativen (z. B. Resektion, Transplantation, Radiofrequenzablation, perkutane Ethanolinjektion) und nicht kurativen Verfahren (z. B. transarterielle Chemoembolisation, Sorafenib-Gabe). Das fibrolamelläre HCC stellt eine Variante des HCC mit anderen klinischen und pathologischen Markern dar.
