A posterior approach to the presacral mass

Abstract

Mass lesions in the presacral space are not readily accessible from the transabdominal approach. The posterior transcoccygeal technique is a nearly ideal approach to the presacral mass, and allows the combined use of abdominosacral resection for more extensive tumors.

Four patients with presacral mass, which later proved to be benign cystic teratomas, each had transcoccygeal operation for resection. The technique gave adequate exposure, satisfactory margins, minimal patient morbidity, and no recurrence of the primary presacral teratomas.

The diagnostic evaluation of the presacral mass and the operative technique of a modified Kraske approach are described with suggestions for resection technique. The utility, flexibility, and limitations of the posterior transcoccygeal approach are reviewed in application to the management of the presacral mass.

Résumé

Les lésions tumorales présacrées sont difficiles à aborder par voie abdominale, La voie transcoccygienne représente la meilleure voie d'accès. Combinée à la voie abdominale, elle permet l'exérèse des tumeurs les plus volumineuses, 4 malades qui présentaient des masses présacrées répondant à des tératomes kystiques ont été opérés par cette voie qui donne un jour excellent grâce à une incision satisfaisante, l'exérèse des tumeurs n'ayant donné lieu ni à complication, ni à récidive.

L'évaluation préopératoire de l'importance de la tumeur présacrée et la technique opératoire de cette modification de la voie de Kraske sont exposées par les auteurs, ainsi que les détails de l'exérèse tumorale. L'utilité, la souplesse et les limites de la voie d'abord transcoccygienne pour pratiquer l'exérèse des tumeurs présacrées sont passées en revue par les auteurs.

Resumen

Las masas ubicadas en el espacio presacro no son fácilmente accesibles por la vía transabdominal. La técnica transcoccígea posterior representa un acceso casi ideal para la masa presacra, y permite el uso combinado de la resección abdominosacra en tumores de mayor extensión.

En una serie de 4 pacientes con masas presacras, que luego probaron ser teratomas quístico benignos, la resección fué realizada mediante la operación transcoccígea. Esta técnica permitió una exposición adecuada con márgenes satisfactorios, y demostró morbilidad mínima y ninguna recurrencia de los teratomas presacros primarios.

Se describen la evaluación diagnóstica de la masa y la operación por medio de la técnica de Kraske modificada, con sugerencias para la resección. La utilidad, flexibilidad y limitaciones del acceso transcoccígeo posterior son revisados en cuanto a su aplicación al manejo de las masas presacras.
