Median nerve entrapment in bone after supracondylar fracture of the humerus

Summary

A case is reported, where the median nerve was entrapped in a supracondylar humerus fracture, not preventing closed reduction. As serious impairment of the median nerve function did not improve within ten weeks, the nerve was explored and dissected out of the humerus using microsurgical technique, leading to complete recovery of the neural function. The purpose of this report is to draw attention to the fact, that the median nerve can be enclosed in supracondylar fractures reducable to good position and further, that delicate dissection of the nerve out of the bone does not necessitate sacrifice of its continuity.

Zusammenfassung

Es wird über einen Fall von suprakondylärer Humerusfraktur berichtet, in die der Nervus medianus eingeklemmt war, ohne jedoch die geschlossene Reposition zu verhindern. Da sich eine erhebliche Beeinträchtigung der Funktion des Nervus medianus innerhalb von 10 Wochen nicht besserte, wurde der Nerv mit mikrochirurgischer Technik exploriert und aus dem Humerusknochen herausgetrennt; im Anschluß daran kam es zu einer vollständigen Erholung der Nervenfunktion. Mit diesem Bericht soll auf die Möglichkeit hingewiesen werden, daß der Nervus medianus in einer suprakondylären Fraktur eingeklemmt sein kann, obwohl es gelingt, diese in gute Stellung zu reponieren. Des weiteren soll gezeigt werden, daß auch eine schwierige Herauspräparation des Nerven aus dem Knochen möglich ist, ohne dessen Kontinuität zu opfern.
