Abstract

Superior hypogastric plexus block (SHPB) involves injection of a local anesthetic or a neurolytic to the superior hypogastric plexus, which lies anterior to the fifth lumbar (L5) vertebra and the first sacral (S1) vertebra, to obtain a sympathetic nerve blocking effect. It is indicated for relief of pain originating from the pelvic viscera. It is frequently implemented by a transdiscal technique or, alternatively, by a paravertebral technique, under X-ray fluoroscopy observation or CT guidance in either case. The paravertebral technique is sometimes necessary because the insertion of the block needle by the transdiscal technique can prove anatomically difficult.
