Abstract
Open rotator cuff repair technique is safe and reproducible and can be done with a mini open technique. A systematic approach including adequate exposure, optimal cuff mobilization, and repair without unnecessary tension is essential for the best chances of success. The surgeon has numerous options for repairing the cuff including transosseous repair and suture anchor repair with either single-row or double-row techniques. There are multitudes of suture repair configurations, many of which work quite well. Key is that the cuff be repositioned as close as possible to its normal anatomical site and that the deltoid origin be repaired so that it heals back to the acromion. Lastly, as with other repair techniques, a supervised postoperative rehabilitation program is vital.
