Video Capsule Endoscopy in Inflammatory Bowel Disease

Abstract

Video capsule endoscopy provides a noninvasive method for the complete endoscopic evaluation of the small bowel mucosa. Its performance has been shown to outperform other imaging modalities for the detection of mucosal lesions. Indications for a video capsule in inflammatory bowel disease (IBD) include the diagnosis of suspected (obscure) small bowel Crohn disease (CD), determination of the extent and severity of small bowel involvement in established Crohn disease, evaluation of unexplained symptoms in established IBD, evaluation of the presence of small bowel lesions in patients with colonic IBD (ulcerative or indeterminate colitis, IBD-U), evaluation of mucosal healing of small bowel Crohn disease after treatment, assessment of possible postoperative recurrence of small bowel Crohn, incomplete colonoscopy, and the assessment of pouchitis. Video capsule in patients with suspected or established Crohn disease is safe, and the results often have a significant clinical impact. However, the foremost contraindication to capsule use is the presence of a known or suspected intestinal obstruction and/or strictures because of the risk of capsule retention. The advent of patency capsules can reduce the risk as a video capsule performed after a positive patency capsule test in patients with Crohn disease is associated with a high risk of video capsule retention. Other practical issues discussed are the use of bowel cleansing preps and prokinetic agents as well as size and age limitations of video capsule use in children.
