Abstract

The American Society of Breast Surgeons (ASBrS) Nipple Sparing Mastectomy Registry (NSMR) is designed to facilitate compiling information on metrics utilized, techniques utilized, aesthetic outcomes, as well as oncologic outcomes of the nipple-sparing mastectomy. We aim to provide a large prospective collection of data points specifically gathered to provide evidence based medicine on outcome measures and metrics utilized for the nipple sparing mastectomy.

Previously the nipple and areola were removed routinely. This was based on a presumption of possible nipple–areola (NAC) involvement, concern for NAC viability, as well as concern regarding detecting recurrence and risk of recurrence. Currently, as a result of basic science studies evaluating breast anatomy, the ductal system, and the nature of terminal ductal lobular units (TDLUs) as well as clinical reports, much more about the nipple is understood. Although NSM is not a new concept, this information has resulted in more cases in which the nipple–areola complex is routinely being spared during mastectomy. This chapter provides a synopsis of the initial findings of the American Society of Breast Surgeons Nipple Sparing Mastectomy Registry including: incidence rate of nipple areolar complex ischemia; sub-areolar tissue specimen assessment; compatibility of breast size, degree of ptosis, type of reconstruction, and incision placement; infectious complication, ptosis and nipple-sparing mastectomy; cup size and nipple-sparing mastectomy; immediate implant reconstruction in nipple-sparing mastectomy; association of incision type and infection rate in nipple-sparing mastectomies; and preliminary oncologic outcome data.
