Abstract

This chapter gives an overview about the philosophy of approaching thoracic disc herniation anteriorly, using either thoracoscopy or mini-open (trans- or retropleural) endoscopically assisted approaches.

Thoracic disc diseases are still considered a big therapeutic challenge because:

  • There are only few studies dealing with the illness’s natural evolution.

  • Their non-characteristic presentation pattern.

  • And the missing evidence comparing surgical and nonsurgical treatments. With this background the surgeon has not only to decide if surgery will benefit the patient but also which approach is the most suitable and less risky for any given case.
