Epidemiology of Respiratory Events in Patients with HM (Not Including ICU)

Abstract

Pulmonary complications in patients with hematological malignancies typically are associated with severe morbidity and frequently result in fatal outcome. The majority of these events are pneumonias caused by microbial pathogens, among which invasive fungal infections are the most life-threatening subgroup. Other microorganisms primarily resistant to broad-spectrum antibacterial agents, particularly viruses, Pneumocystis jiroveci, mycobacteria and multi-resistant bacteria, must be addressed by adequate diagnostic procedures, but important differential diagnoses apart from infectious diseases must be taken into consideration as well, first of all pulmonary side effects from cytotoxic drugs, radiotherapy or pulmonary involvement by the underlying malignancy. Clinical outcome may be favorable also in patients with severe respiratory failure, so that intensive care referral and invasive cardiorespiratory intervention are ethically justified in patients whose clinical course is not expected to be unequivocally fatal.
