Nella classificazione delle cefalee della International Headache Society (IHS) del 1988 [1] il gruppo 4, denominato “Cefalee varie non associate a lesioni stratturali”, comprendeva sei differenti tipi di cefalea: la cefalea trafittiva idiopatica, la cefalea da compressione esterna, la cefalea da stimolo freddo, la cefalea da tosse benigna, la cefalea da sforzo benigna, la cefalea associata ad attività sessuale. Nella nuova revisione della classificazione (ICHD-II) del 2004 il gruppo 4 [2], denominato “Altre cefalee primarie”, include otto differenti tipi di cefalea. Rispetto all’edizione del 1988, due forme (la cefalea da compressione esterna e la cefalea da stimolo freddo) non compaiono più perché sono state spostate nel gruppo 13 “Nevralgie craniche e cause centrali di dolore facciale”, ma sono inserite quattro nuove entità cliniche: la cefalea ipnica, la cefalea a rombo di tuono primaria, la hemicrania continua e la new daily persistent headache (NDPH); inoltre, gli aggettivi idiopatica e benigna sono stati sostituiti dall’aggettivo primaria.
