Zusammenfassung

Die Opioide sind die stärksten und sichersten Analgetika die wir haben und werden dennoch auf Grund von Vorurteilen, Unwissenheit und Mythen in vielen Ländern von der Gesetzgebung benachteiligt und in der Folge auch von verschreibenden Ärzten zu restriktiv verordnet. Opioide zeigen ein Phänomen, das in dieser Art bei anderen Arzneimitteln nicht zu beobachten ist. Titriert man einen Schmerzpatienten bis zur Schmerzfreiheit, vor allem auf oralem Weg, ist die Gefahr einer Atemdepression gering. Titriert man darüber hinaus oder gibt man dieselbe Dosis einem Gesunden, kann eine bedrohliche Atemdepression eintreten. Ist ein Schmerzpatient gut auf Opioide eingestellt und man beseitigt den Schmerz mit einer anderen Methode, z. B. einer Blockade und verabsäumt, die Opioid-Dosis zu vermindern, ist ebenso eine Atemdepression die Folge.
