Zusammenfassung

Länger andauernde und subjektiv belastende körperliche Beschwerden, die medizinisch nicht ausreichend erklärt werden können, werden auf Ebene der fachärztlichen Versorgung häufig als funktionelle somatische Syndrome bezeichnet. Im psychiatrischen und psychosomatischen Kontext wird häufiger die Diagnosekategorie der somatoformen Störungen verwendet. In Bezug auf die Ätiopathogenese dieser Störungen ist eine biopsychosoziale Betrachtung unbedingt angezeigt. Das Zusammenspiel möglicher genetischer, neuroendokriner sowie hirnanatomischer und -funktioneller Veränderungen auf der einen Seite mit psychosozialen Faktoren wie stressreichen Lebensereignissen, Stresserleben im Alltag und schlechtem sozioökonomischen Status auf der anderen Seite muss bei der individuellen Entstehung und Aufrechterhaltung beachtet werden. Neben somatomedizinischen und verhaltensmedizinischen Ansätzen stellt nach Studienlage die kognitiv-behaviorale Psychotherapie, z. T. in Kombination mit der Gabe von Antidepressiva, die Behandlung der Wahl dar.
