Zusammenfassung

Einer ausgeprägten arteriosklerotischen Läsion ist nicht anzusehen, welche Schritte in ihrer Entwicklung abgelaufen sind und welche das Geschehen initiiert haben. Wir wissen allerdings, daß die Länder mit hohem durchschnittlichen Plasmacholesterinspiegel eine hohe Rate an koronarer Herzkrankheit haben, während die Länder mit niedrigem Gesamtcholesterin von 160 mg/dl oder darunter dieses Problem nicht kennen, trotz oftmals erheblicher Prävalenz anderer Risikofaktoren wie Hypertonus und Rauchen. Daher muß dem Plasmacholesterinspiegel eine zentrale Rolle zukommen [1]. Während die experimentelle Arterioskleroseforschung lange Zeit davon ausging, daß es primär zu einer Endothelläsion kommen müsse, wissen wir heute, daß der erste Schritt bei intaktem Endothel ablaufen kann und kompliziertere Vorgänge beinhaltet als bloße Insudation von Cholesterin in die Arterienwand.
