Zusammenfassung
Ausmaß und Intensität von Schlafstörungen bei Epilepsien korrelieren i. allg. sowohl mit der Schwere des Anfallsleidens als auch mit der Ausprägung begleitender neurologischer Ausfälle. Während sich bei Anfallsleiden geringer Intensität — etwa der benignen Epilepsie des Jugendalters mit Rolandischen oder temporalen Spitzen — oft keine Schlafstörungen nachweisen lassen [6, 14], sind bei Patienten mit multiplen Grand-mal-Anfällen im Schlaf die Schlafzyklen oft bis zur Unkenntlichkeit zerstört. Auch bei komplex-fokalen Anfällen korreliert das Ausmaß der Schlafstörung in etwa mit der interiktualen Spike-Dichte [32]. Nach tonisch-klonischen Anfällen [10] und nach komplex-fokalen Anfällen [4] im Schlaf ist die REM-Schlafmenge gewöhnlich vermindert. Bei Patienten mit generalisierten oder therapieresistenten Anfällen scheinen Schlafstörungen häufiger zu sein als bei fokalen Epilepsien. Charakteristische Symptome einer Schlafstörung bei Epilepsie zeigt die folgende Übersicht:

verlängerte Einschlaflatenz,

gehäuftes Erwachen während des Nachtschlafes,
-
verminderte Schlafeffizienz,
-
Zunahme der NREM-Stadien 1 und 2,
-
Abnahme der Tiefschlafstadien NREM 3 und 4,
-
verminderte Dichte von Schlafspindeln und K-Komplexen,
-
verminderter und fragmentierter REM-Schlaf,
-
Zunahme der REM-Latenz,
-
vermehrte Häufigkeit der Schlafstadienwechsel [58].
