Chronische Obstipation und Stuhlinkontinenz

Zusammenfassung

Daß psychosoziale Faktoren bei der Defälcation — und damit auch bei Defäkationsstörungen — eine Rolle spielen können, erscheint auf den ersten Blick offensichtlich: Dem Kleinkind wird mit Akribie die regelrechte und kontrollierte Dannentleerung zum frühestmöglichen Zeitpunkt anerzogen („Toilettentraining“); spätestens mit Erreichen dieses Ziels jedoch gelten der Defäkationsvorgang und seine Unregelmäßigkeiten als tabu, so daß Patienten mit Stuhlinkontinenz selbst ihrem behandelnden Arzt gegenüber ihre Beschwerden nicht oder nur zögernd berichten [20]. Trotzdem — oder gerade deswegen — wird dem Erwachsenen ständig und nachhaltig die Wichtigkeit einer regelmäßigen, fälschlicherweise „gute Verdauung“ genannten Darmentleerung durch die Werbung der Hersteller von Abführmitteln und anderen „Darmregulanzien“ suggeriert. Sprachliche Assoziationen (Durchfall = Versagen, z.B. in Prüfungssituationen) reflektieren gelegentlich vermutete Zusammenhänge zwischen psychosozialen Faktoren und Darmfunktionsstörungen aus der Alltagserfahrung. Ob und welche Faktoren demgegenüber die Dickdarmfunktionen tatsächlich beeinflussen, war Forschungsgegenstand der Psychologie, Psychosomatik und Psychophysiologie in den vergangenen 100 Jahren. Im folgenden sollen populäre Konzepte über diesen Zusammenhang zwischen Defäkation/Defäkationsstörungen und psychischen und sozialen Faktoren kritisch diskutiert werden.
