This paper presents first clinical experiences with the rotation angioplasty using the Viadyn and with the rotation arterectomy with the Atherodyn in the Department of Vascular Surgery, Clinic for Surgery, District Hospital Halle.
From January 1,1992 — September 15,1992 these methods were used in 33 patients with peripheral artery disease (PAD) undergoing surgery. There were 23 patients with PAD stage IV (Fontaine scale), 20 of them older than 60 years. All these patients had additional diseases; 12 of these were diabetic.
In 12 patients out of these 23 the clinical situation could be improved and the typical symptoms of stage IV disappeared. In 11 patients the rotation methods were not successful. Therefore in nine patients we had to amputate this extremity. In two patients out of these 11 we could save the extremity using intraarterial infusion of prostaglandin E1.
These methods, performed by a vascular surgeon, offer the possibility to save an extremity of a geriatric polymorbid patient with PAD under minimal invasive operative conditions in the event that an established surgical method is not suitable.
