Abstract
This study will contribute to clarifying the problem of the fractured neck of the femur. The leading idea is the recognition of the fact that mechanical forces can gain a decisive influence on the biology of bony tissue. The healing of a fracture is only possible if, at each stage of the evolution, the capacity for mechanical resistance of the tissue of regeneration is sufficient to resist the mechanical stressing. It is on a similar theory that the definition of the essential nature of a pseudarthrosis given by Walter Müller 1 is based: “The state of the pseudarthrosis is the result of a disproportion between the capacity of the bone to form callus and the mechanical changes.”

This is then supported as by a chin but the main thing is that the head is now supported directly by the shaft, whereas the pseudarthrosis is completely remote from loading. The line of action of the loading is now much straighter through the shaft and the head is interposed as a ball bearing between pelvis and femoral shaft.”

The X-ray picture of the bone is taken from the work of A. Kolodny: Architecture and blood supply of the head and neck of the femur and their importance in the pathology of fractures of the neck. J. Bone Jt Surg. 23, 1925, p. 575. The X-rays taken as usually in lying with the feet joined and pointing upwards show correctly the direction of the trabeculae. Moderate changes in the position of the foot provoke no significant alteration in the direction of the trabeculae structure, as shown experimentally.
