Zusammenfassung

EKT wirkt antidepressiv, antimanisch, antipsychotisch, antikonvulsiv, antisuizidal, stimmungsstabilisierend und antikataton. Ihre Wirksamkeit bei depressiven Erkrankungen ist durch zahlreiche Studien belegt. Bei einer schweren depressiven Episode mit psychotischen Symptomen ist sie der Psychopharmakatherapie überlegen. Bei depressivem Stupor, Nahrungsverweigerung oder akuter Suizidalität im Rahmen einer depressiven Erkrankung kann eine vitale Indikation zur EKT-Behandlung vorliegen. Katatone Symptome sprechen unabhängig von der Grunderkrankung gut auf EKT an. In der Behandlung der perniziösen Katatonie ist sie die Behandlungsmethode der 1. Wahl. In der Schizophreniebehandlung und der Behandlung von schizoaffektiven Störungen stellt die EKT grundsätzlich eine augmentative Behandlung zur Therapie mit Antipsychotika dar. Bei echter Therapieresistenz der genannten Krankheitsbilder ist sie immer eine Alternative.
