Abstract
The Patient Centered Medical Home is an evidence-based well-defined care delivery model that is considered to be one of the most promising care delivery systems to improve patient outcomes, the cost of health care, and access to care. It is thought to be the solution for the US health care crisis. Over the last few years, numerous demonstration projects have been funded by various organizations to help convert practices into a PCMH model of care. The results of these demonstration projects supported the effectiveness of the PCMH.
Even though it is still evolving, the PCMH is an exemplary evidence-based care delivery system characterized by high-quality, low-cost, collaborative/interdisciplinary care that is coordinated across multiple health care systems. As it evolves and expands, more practitioners including nurse practitioners (NPs) are needed to assume a lead role in providing primary care to patients. NPs have all the necessary attributes to assume the role of the primary care provider.
