Instrumentation in the Childhood Spinal Deformities: Challenges, Problems, Limitations, and Solutions

Abstract

Although spinal instrumentation is performed on a larger number of children today, the unique characteristics of their spines present a number of problems still. While advanced technology has mostly solved those related to implant profile, inadequate purchase of screws in the largely cartilaginous pediatric spine, the impact of implants and fusion on ongoing growth, and problems related to reactions caused by the materials used in the manufacturing of spinal implants still pose challenges to the spinal surgeon. The potential negative effect of the evermore popular pedicle screw on the neurocentral cartilage should also be considered. This chapter reviews these challenges unique to the growing spine and offers possible solutions.
