Secondary Rhinoplasty:Surgical Techniques

Abstract

Can secondary rhinoplasty be taught? Three inescapable facts say no: (1) the highly variable normal nasal anatomy is often destroyed by the earlier surgery and distorted by scar contracture, (2) operative plans must be changed radically when intraoperative findings dif-fer markedly from preoperative expectations, and (3) the diversity of cases makes learning surgical cause and effect difficult. Ultimately, secondary rhinoplasty must be based on the fundamental principles of primary rhinoplasty. One simply cannot provide a formula for dealing with the complexities of secondary rhinoplasty. Just as there is a progression from medical school through residency to fellowship, most surgeons should progress from primary rhinoplasty through their own revisions to secondary rhinoplasty over a 3–5-year period. Note: a secondary rhinoplasty is defined as a case where the primary rhinoplasty was per-formed by another surgeon; a revision is when you reoperate on your own primary case.
