Leitlinien haben die Aufgabe, die gemeinsame Entscheidungsfindung von Ärzten und deren Patienten zu einer im Einzelfall sinnvollen gesundheitlichen Versorgung zu unterstützen. Dabei unterscheiden sich die Entscheidungssituationen im hausärztlichen Versorgungsbereich von denen anderer Settings, da Hausärzte unselektierte Patienten mit verschiedensten gesundheitlichen Problemen über einen langen Zeitraum hinweg versorgen. Diese Besonderheiten müssen von einer hausärztlichen Leitlinie berücksichtigt werden. Darüber hinaus stellt sich die Frage, inwiefern bereits existierende internationale Leitlinien auf die deutsche Versorgungssituation mit ihren kontextuellen Besonderheiten übertragbar sind, da die Neuentwicklung von evidenzbasierten Leitlinien mit erheblichem Zeit- und Personalaufwand verbunden ist.
Das folgende Kapitel beschreibt wesentliche Stationen im Entwicklungsprozess der hausärztlichen DEGAM-Leitlinie zur Versorgung der chronischen Herzinsuffizienz, in dem in einigen Abschnitten Neuland betreten wurde. So wurde erstmals ein Verfahren (der sog. systematische Leitlinienreview) angewendet, mit dem existierende internationale Leitlinien zu diesem Th ema systematisch analysiert und hinsichtlich der inhaltlichen Angemessenheit ihrer Empfehlungen überprüft wurden. Bereiche mit unklarer Evidenzlage wurden für weiterführende Recherchen identifiziert. Nach Abschluss dieser Recherchen und damit verbundenen Evidenzbewertungen sowie interner und externer Begutachtungen des Leitlinienentwurfs, wurde dieser formal und interdisziplinär konsentiert. Erstmals in Deutschland wurden bei der Anwendung der Technik des Nominalen Gruppenprozesses auch quantitative Methoden eingesetzt, um Dissens aufzuspüren und im weiteren Verlauf den Grad an Übereinstimmung abzubilden.
Im Ergebnis des vom BMBF industrieunabhängig geförderten Entwicklungsprozesses entstand eine umfassende, evidenzbasierte, formal konsentierte und praxisgetestete hausärztliche Leitlinie zur Versorgung der chronischen Herzinsuffizienz. Die besondere Vorgehensweise in der Evidenzgewinnung war ressourcensparend — indem sie unnötige Repetitionen in der Evidenzbewertung vermied — und dennoch valide, indem entscheidungsrelevante Evidenzbezüge einer Neubewertung unterzogen wurden. Dabei, wie auch im Nominalen Gruppenprozess, wurden setting- und kontextspezifische Aspekte in besonderem Maße berücksichtigt. So wurde bspw. — abweichend von spezialärztlichen Leitlinien — eine klare Negativempfehlung in der DEGAM-Leitlinie für den Einsatz von Tests auf Brain Natriuretic Peptides ausgesprochen, da belastbare Evidenz für das prim→medizinische Setting fehlte. Im Gegensatz zum Entscheidungskontext vieler internationaler Leitlinien wurden ß-Rezeptorenblocker nicht generell für alle asymptomatischen Herzinsuffizienzpatienten empfohlen, sondern nur nach durchgemachtem Myokardinfarkt. Nur diese Subpopulation war in Studien ausreichend repräsentiert, und bei fehlendem Wirksamkeitsnachweis für andere Subpopulationen stand das nonmaleficence-Prinzip bei der Formulierung der Empfehlung im Vordergrund.
Diese Leitlinie wurde im Herbst 2006 als DEGAM-Leitlinie Nr. 9 autorisiert und veröffentlicht [46, 47]. Zwischenzeitlich wurde sie einer unabhängigen Bewertung durch das Ärztliche Zentrum für Qualität in der Medizin (ÄZQ) unterzogen. Aufgrund ihrer hohen methodischen Qualität wurde sie als Quellleitlinie für die Nationale Versorgungsleitlinie zur chronischen Herzinsuffizienz ausgewählt (http://www.versorgungsleitlinien.de/themen/herzinsuffizienz).
