Ataxies sporadiques tardives d’étiologie indéterminée (ILOCA, SAOA)

Résumé

Les ataxies sporadiques tardives d’étiologie indéterminée (ILOCA: idiopathic late-onset cerebellar ataxia) sont aussi nommées SAOA (sporadic adultonset ataxia of unknown etiology) [1, 2]. Elles se différencient de la MSA de type C, même si ces entités pourraient constituer les extrémités d’un même spectre dans certains cas. En effet, les patients présentant une ILOCA ne répondent pas aux critéres de MSA (voir chapitre 15). Les tests génétiques actuellement disponibles demeurent négatifs. Par définition, les causes connues d’ataxie sporadique doivent être exclues pour envisager un diagnostic d’ILOCA. Compte tenu de l’absence de marqueurs biologiques ou radiologiques de la maladie, il ne peut donc s’agir que d’un diagnostic d’exclusion.
