Résumé
Les maladies inflammatoires chroniques et cryptogéniques de l’intestin (MICI) sont des affections évoluant par poussées et touchant avec prédilection les adolescents et les adultes jeunes. Les formes pédiatriques représentent toutefois 10 à 15% des cas, le plus souvent au-delà de 10 ans. Elles touchent plus d’un sujet sur 1000 dans les pays occidentaux et leur fréquence augmente rapidement dans les pays relativement épargnés jusqu’à maintenant. Elles résultent d’une anomalie de la réponse immunitaire de l’intestin vis-à-vis de composants de la flore bactérienne survenant chez des sujets génétiquement prédisposés. Elles comprennent deux entités bien individualisées, la rectocolite hémorragique (RCH) et la maladie de Crohn (MCr) (tableau 79.1). Dans certaines formes atypiques limitées au côlon et oú l’histologie est peu évocatrice, la distinction entre les deux peut être impossible; on parle alors de colites inclassées. Le bilan diagnostique minimum à réaliser en cas de suspicion deMICI est le suivant: hémogramme, VS, CRP (une augmentation doit attirer l’attention), anticorps anti-saccharomyces cerevisiae (ASCA), anticorps anti-cytoplasme des neutrophiles (ANCA), fibroscopie oeso-gastro-duodénale et iléocoloscopie avec biopsies systématiques.
