Abstrait

L’abord « chirurgical » du thorax permet la chirurgie du cœur, des gros vaisseaux, de l’œsophage et des autres éléments contenus à l’intérieur du thorax ou le constituant. La chirurgie cardiaque ne concerne qu’exceptionnellement des tumeurs. La chirurgie des cancers de l’œsophage a été traitée dans un autre chapitre de cet ouvrage. La chirurgie thoracique, dont les indications pour cause tumorale sont les plus fréquentes, s’adresse aux autres structures intrathoraciques: trachée, bronches, poumon, médiastin et plèvre. Elle s’adresse é galement à la paroi du thorax elle-même. Rares, et le plus fréquemment observées chez le sujet jeune, les tumeurs de la paroi thoracique peuvent aussi toucher le sujet âgé (fig. 1). Chez celui-ci, elles sont plus souvent malignes. Elles nécessitent parfois le recours à des gestes chirurgicaux majeurs d’exérè se et de reconstruction pariétale (fig. 2). L’âge n’est plus une contre-indication chez un patient par ailleurs en bonne santé (1).
