Abstract
Mitral valve(MV) repair for severe mitral regurgitation (MR) secondary to degenerative MV disease is an accepted treatment for patients with symptoms, and for those without symptoms but with decreased left ventricle (LV) systolic function or an increase in left ventricular end systolic dimension. Successive clinical guidelines, over recent years, have suggested that surgery should be offered increasingly early in the disease process. The most recent ACC/AHA guideline suggests that it is reasonable to offer mitral repair to asymptomatic patients with severe MR and normal left ventricular size/function as long as the center is experienced in repair and likelihood of successful repair is high. This chapter will explore the evidence behind these recommendations, and their potential implications.
