Abstract
Ischemic heart disease remains the leading cause of death in the Western Hemisphere. Despite recent advances in medical therapy and surgical and percutaneous revascularization, the number of so-called no option patients is increasing. These patients are no longer candidates for percutaneous or surgical revascularization but are symptomatic despite maximal medical therapy. A number of investigational therapeutic strategies have been developed to improve the quality of life of “no-option” patients. These, in addition to laser myocardial revascularization discussed here, include angiogenesis and myogenesis approaches, external counterpulsation, as well as spinal cord stimulation.
