Abstract

Deep venous thrombosis or documented thromboembolism in a patient who has a contraindication to anticoagulation. Recurrent thromboembolism despite adequate ­anticoagulation. Complications of anticoagulation that required discontinuation of therapy. Immediately after pulmonary embolectomy. Failure of another form of caval interruption, demonstrated by recurrent pulmonary thromboembolism.
