Abstract
Minimal incision endoscopic midface rejuvenation allows elevation and volume augmentation of the midface in a safe and effective manner. I have performed and refined this technique over a 20-year period with excellent results and few complications. The surgery is performed in the subperiosteal plane and can be augmented with the addition of facial implants, fat grafting, and laser or chemical skin resurfacing. The critical element of surgery is the imbrication and anchoring of various tissues (suborbicularis oculi fat, Bichat’s fat pad, etc.) which synergistically project the cheek, and efface the tear trough and the nasolabial fold in a closed surgical approach. The procedure is appropriate for the young and old and yields substantial results without obvious signs of surgery.
