Abstract

The transseptal puncture is a critical part of the MitraClip procedure. It is performed under transesophageal echocardiographic guidance because precise localization of the puncture site is crucial for the rest of the procedure. The location of the puncture determines the maneuverability of the MitraClip guide and the clip delivery system (CDS). A properly selected puncture site allows for a smooth and uncomplicated procedure, while a poorly selected location will result in difficulties in positioning the MitaClip and a prolonged procedure. In addition, transseptal puncture is also associated with risks of inadvertent puncture of the aorta, perforation of the atrium, and pericardial tamponade. This chapter reviews the key steps in the transseptal puncture for the MitraClip procedure.
