Abstract

The author discusses the importance of developing a leadership structure for your CPOE project. He also lays out the many policies and procedures that a hospital should contemplate as they prepare for their CPOE deployment. The hospital should determine early in their project as to whether to require physicians to comply with mandatory training and CPOE use.

    e-Prescribing (the electronic routing of prescriptions from provider to pharmacy) has enabled the pharmacies and pharmacy benefits managers (PBM) to establish a robust database linking chain and independent pharmacies, as well as large medication distribution centers and PBM. Currently their database, incorporating SureScripts and RxHub, captures about 90 % of the prescribed medications in the U.S. However, patients have other sources of medications, including samples, non-participating pharmacies (both domestic and abroad) in addition to well meaning families and friends. In addition, regional health information organizations (RHIO) and health information exchanges (HIE) improve access to medication histories, but are not the complete “source of truth”.

    Polypharmacy refers to the practice of placing patients on large number of medications, ­increasing the risk of drug-drug interactions and mishaps.
