Abstract

With the improvements in the diagnosis and management of head and neck cancer (HNC), there appears to be a shift in the pattern of failure. Better locoregional control is now achievable with combined modality therapy, and this has translated into a higher rate of observed distant metastasis. A similar shift in the pattern of failure over time has also been noted in oral cavity cancers. In addition, patients who develop distant metastasis have an extremely poor prognosis, with only few long-term survivors. A better understanding of the molecular pathways governing tumor dissemination is critical for the treatment and prevention of metastases in oral cavity cancers. Both tumor hypoxia and angiogenesis have been implicated in the development of distant metastasis in solid cancers, specifically HNC. Both processes mediate the induction of several genes that have been shown to modulate cellular properties critical for tumor dissemination. In this chapter, we review the role of hypoxia and angiogenesis in the development of metastasis, with a focus on oral cavity cancer.
