Zusammenfassung
Das peptische Ulkus ist, vom medikamenten-induzierten Ulkus abgesehen, ein chronisches Leiden, das in Schüben verläuft und den Betroffenen über Jahre und Jahrzehnte begleitet, wenn es nicht gelingt, die entscheidenden pathogenetischen Faktoren auszuschalten. Dabei ist das Durchschnittsalter der Ulcus-duodeni-Patienten im allgemeinen 10 Jahre niedriger als beim Ulcus ventriculi. Ulzera im operierten Magen treten bevorzugt bei Rauchern auf, die wegen eines Ulcus duodeni operiert wurden.
Sieht man von seltenen Ursachen des Ulkusleidens wie Gastrinom (Zollinger-Ellison- Syndrom) und primärem Hyperparathyreoidismus ab, steht die Helicobacter-pylori-Infektion (H.p.) der Magenmukosa bzw. die chronische Typ-B-Gastritis ganz im Vordergrund des pathogenetischen Geschehens, gefolgt von Mukosaschäden durch ASS und nichtsteroidale Antirheumatika (NSAR). Mit Sanierung der H.p.-Infektion verschwindet die Rezidivneigung; das Ulkus heilt aus.
