Zusammenfassung
Die Therapie des Asthma bronchiale befindet sich im Wandel. Die Stellung von inhalativen Steroiden als antiinflammatorische Dauertherapie ist nunmehr gesichert. Langwirkende inhalative β2-Agonisten haben ihren Platz im Stufenschema der Asthmatherapie gefunden, kurzwirkende inhalative β2-Mimetika ergänzen die Therapie auf allen Stufen als Bedarfsmedikation. Die aktuelle Diskussion bezieht sich auf die seit einigen Jahren verfügbaren Leukotrienrezeptorantagonisten und auf das so alte Theophyllin, das in niedriger Dosis zusätzlich einen gewissen antientzündlichen Effekt aufweist. Neuartige Therapieansätze befinden sich in der klinischen Prüfung (Phosphodiesterase-4-Hemmer, Anti-IgE), deren Stellenwert für die Therapie des Asthma bronchiale derzeit noch nicht abschätzbar ist (Giembycz 2000; Busse 2001). Hinsichtlich der Umsetzung bestehender Therapieempfehlungen besteht leider noch Nachholbedarf (Rabe et al. 1999).
