Zusammenfassung
Die sympathikotone orthostatische Hypotonie ist eine prognostisch harmlose Kreislaufregulationsstörung, die asympathikotone orthostatische Hypotonie eine prognostisch ernste Teilproblematik im Rahmen von zumeist neurologischen Systemerkrankungen. Schellong-Test und, bei asympathikotoner Hypotonie, autonome Funktionsteste sind diagnostisch einzusetzen. Eine spezifische Therapie der sympathikotonen orthostatischen Hypotonie ist häufig unnötig, eine wirksame Therapie der asympathikotonen Formen nur unzureichend möglich.
Synkopen sind ein häufiges Problem und können je nach Ursache eine banale Ohnmacht sein oder Vorläufer des plötzlichen Herztodes. Bei Herzgesunden sind sie zumeist autonom-nervaler (vasovagaler) Genese, bei Vorliegen einer strukturellen Herzerkrankung häufig rhythmogen bedingt. Diagnostisch sind die Kipptischuntersuchung bei Verdacht auf neurokardiogene Synkope, die elektrophysiologische Untersuchung oder die Langzeitrhythmusüberwachung (implantierbarer Loop-Recorder) bei Verdacht auf rhythmogene Synkope wegweisend. Mechanisch obstruktive Synkopen sind zumeist leicht zu erkennen mittels (Doppler-) Echokardiographie. Neurokardiogene Synkopen werden mittels Patientenaufklärung, Stehtraining, erhöhter Flüssigkeitszufuhr und ggf. medikamentös behandelt. Bei den prognostisch ernsten rhythmogenen oder mechanisch obstruktiven Synkopen wird die zugrunde liegende Rhythmusstörung bzw. Erkrankung gemäß aktuellen Empfehlungen therapiert. Aktuelle Leitlinien zur Diagnostik und Therapie von Synkopen wurden 2001 von der European Society of Cardiology veröffentlicht (Task Force on Syncope 2001).
