Resumen
Objetivo
Evaluar la eficiencia de nimotuzumab en el tratamiento de los pacientes diagnosticados de cáncer de cabeza y cuello incluidos en el ensayo clínico (EC).
Métodos
Se analizó retrospectivamente la base de datos de los pacientes incluidos en el EC controlado, aleatorizado y doble ciego. En el mismo un grupo de pacientes recibió el nimotuzumab más radioterapia (RT) y el grupo control recibió placebo más RT. Teniendo en cuenta las bases y las historias clínicas de estos pacientes, se realizó un análisis de coste-efectividad medio (ACEM), de coste-efectividad incremental (ACEI) y análisis de sensibilidad en ambos grupos. Para esto se tuvieron solamente en cuenta los costes directos asociados al tratamiento con nimotuzumab o con placebo.
Resultados
Los años de vida ganados (AVG) por paciente con nimotuzumab fue de 1,71 frente a 1,44 con el grupo control. El ACEM en el grupo de estudio fue 2.499,37 $/AVG por paciente en comparación al placebo (1.658,73 $ AVG). Con el cálculo del ACEI el beneficio extra en salud que se obtiene con la administración de nimotuzumab compensa el alto coste del mismo, 5.402,1 $/AVG por paciente.
Conclusión
A pesar de que el tratamiento con el monoclonal en pacientes con cáncer de cabeza y cuello es más costoso, su alta efectividad terapéutica lo convertirá en una opción más eficiente en términos incrementales, porque se obtiene un beneficio adicional en la supervivencia de los pacientes con un razonable coste para la economía nacional. Todos los pacientes mostraron una mejora en la calidad de vida y una reducción de los síntomas generales y específicos de la enfermedad.
Abstract
Objective
To evaluate the efficiency of nimotuzumab in the treatment of the patients diagnosed with squamous cell carcinoma of the head and neck (SCCHN) included in the clinical trial.
Methods
It was analyzed retrospectively the database of the patients included in the double-blind, placebo-controlled, randomized clinical trial. In this study, control patients received a placebo plus radiotherapy (RT). From the records of this clinical trial, cost-effectiveness analyses (CEA), incremental cost-effectiveness ratio (ICER) and sensibility analysis in both group were calculated. Direct costs of the treatment with nimotuzumab/placebo were taken into account.
Results
Life years gained (LYG) per patient with the nimotuzumab was 1.71 vs 1.44 years in the control group. CEA of the study group was $ 2 499.37/LYC per patient in comparisson placebo group ($ 1 658.73/LYG). Nevertheless, in the ICER the extra benefit in health of the nimotuzumab compensated the higher level of cost incurred to obtain an efficiency of $ 5 402,1/LYG additional per patient.
Conclusion
Although the treatment with monoclonal antibody in patients with SCCHN was more expensive, its higher therapeutic effectiveness made it the most efficient alternative, because it was obtained additional benefit in the survival of the patients, with a reasonable cost for national economy. All patients showed a quality of life improvement and a reduction of the general and specific symptoms of the disease.
