Resumen
Objetivos
Evaluar el coste-efectividad a largo plazo de ticagrelor más ácido acetilsalicílico en comparación con clopidogrel más ácido acetilsalicílico en pacientes con síndrome coronario agudo (SCA) tratados durante 12 meses en España.
Métodos
El modelo de coste-efectividad se compone de un árbol de decisión (primer año) basado en el estudio PLATelet Inhibition and Patient Outcomes y un modelo de Markov para el largo plazo (segundo año en adelante), que estimó eventos cardiovasculares (muerte, infarto de miocardio e ictus no mortal), coste sanitario y calidad de vida relacionada con la salud durante toda la vida del paciente. El coste diario farmacológico fue de 0,60 € para clopidogrel genérico y 2,96 € para ticagrelor. Se descontaron los costes (€ de 2013) y los efectos al 3%. Se realizó un análisis de sensibilidad por subgrupos y un análisis probabilístico para validar la robustez del modelo.
Resultados
Ticagrelor se asoció con ganancias de 0,1586 años de vida y de 0,1363 años de vida ajustados por calidad (AVAC), con un coste incremental de 596 €. El coste por año de vida ganado fue de 3.760 € y por AVAC de 4.374 €. El análisis de sensibilidad probabilístico confirmó que ticagrelor fue coste-efectivo en más del 99% de las simulaciones frente a clopidogrel, con una disponibilidad a pagar de 15.000 € por AVAC. Los resultados fueron consistentes entre los distintos subgrupos con SCA.
Conclusiones
Ticagrelor más ácido acetilsalicílico es una alternativa coste-efectiva comparada con clopidogrel más ácido acetilsalicílico en pacientes con SCA tratados durante el primer año, según los datos del estudio PLATelet Inhibition and Patient Outcomes y los costes españoles.
Abstract
Objectives
To evaluate the long-term cost-effectiveness of ticagrelor plus aspirin versus clopidogrel plus aspirin in patients with acute coronary syndrome (ACS) treated for 12 months in Spain.
Methods
The cost-effectiveness model consisted of a decision tree (first year) based on the PLATelet Inhibition and Patient Outcomes study and a long-term Markov model (second year onwards). This allowed estimation of cardiovascular events (death, myocardial infarction, and non-fatal stroke), survival, health costs, and health related quality of life. A life time horizon was applied. The daily drug cost of generic clopidogrel was €0.60 versus €2.96 for ticagrelor. Spanish unit costs and life tables were used; outcomes and costs were discounted at 3%. A sensitivity analysis across subgroups was carried out, and a probabilistic sensitivity analysis was used to validate the robustness of the model.
Results
Compared to clopidogrel, ticagrelor was associated with a gain of 0.1586 life years and 0.1363 quality-adjusted life years (QALYs), with an incremental cost of €596. The incremental cost per life year and per QALY gained €3,760 and €4,374, respectively. A probabilistic sensitivity analysis showed that ticagrelor was cost-effective versus clopidogrel in >99% of the simulations, with a willingness-to-pay threshold of €15,000 per QALY. The results were consistent across different subgroups of patients.
Conclusions
Ticagrelor plus aspirin for 12 months is a cost-effective treatment compared to generic clopidogrel plus aspirin in patients with ACS, based on the findings of the PLATelet Inhibition and Patient Outcomes study and Spanish health care costs.
